A New Title Form needs to be submitted for every new addition to a reading list. Below are the forms in two different formats. Do a "save as" to save a blank copy to your own computer.
New Title Form in PDF Format - This can be printed and filled in by hand, and mailed in to Betsy Ginsburg. 
New Title Form in Word document format. You can fill this out on your computer and submit it as an attachment in an email. Save it first to your own computer, then everytime you use it do a "save as" again to preserve the blank form.
